About the Job
Responsibilities and Duties: (65%) Care coordination for patients; Support MOUD providers with addiction medicine visits:
- Meet with all new patients during the induction to review the program's forms and offer care coordination for resources or support.
- Maintain an active suboxone patient panel within the EMR. Contact patients when prescriptions refills are due, and appointments are needed.
- Actively interact with patients who are experiencing a crisis. Use active listening to allow the patient to express their frustrations and validate their feelings. Apply solution-focused strategies to work with patients to create plans that meet their needs.
- Assist with managing all new referrals to the suboxone program, which may include registering new patients, explaining program guidelines and expectations to the patient, and scheduling appointments.
- Manage internal referrals through the EMR by reaching out to the patient and scheduling as appropriate.
- Available for warm hand-offs from providers to meet with patients who are still in the pre-contemplative or contemplative stage with their substance use. Use motivational interviewing skills to assist the patient in making an informed decision.
- Complete and submit prior authorization requests to insurance providers for prescribed medications, suboxone and others.
- Contact pharmacies, as needed, for patients when issues arise regarding any of their prescribed medications.
- Serves as the main contact person in the clinic for patients enrolled in the suboxone program.
- Updates care team, as needed, when any changes are made to the patient's care plan and document the changes in the EMR.
- Monitor transportation concerns and other access barriers for patients.
- Coordinate with outside agencies (i.e., treatment centers, probation officers, CPS workers, etc.) to support the patients.
- Assist patients in understanding the healthcare and managed care delivery system and act to resolve obstacles that prevent them from receiving adequate mental health care.
- Assist patients with following up on any external referrals to specialty services.
- Meet with patients who are actively in withdrawal and looking for detox services. Help with finding resources/open beds in outside facilities and coordinating admission as able.
- Provides information and resources to patients who are seeking treatment (residential or outpatient). Assists patients with scheduling if they have phone insecurity.
- Provide counseling to patients experiencing acute mental health/addiction crisis.
- Support patients through transitions of care (hospital to clinic, emergency room to clinic, clinic to treatment center).
(20%) Clinical research support; Documentation for reporting:
- Assist clinical research team with delivery of behavioral activation intervention and contingency management program.
- Meet with patients being seen by medical providers within the clinic to provide brief interventions for emotional health of social problems.
- Works directly with patients to set care goals and help create treatment plans including psychological and mental health needs.
- Documents appropriate information in the patient's medical record to ensure communication of the patient's needs to the treatment team.
- Document required program data, as needed, for reporting purposes.
(10%) Community engagement and networking with partners; maintaining list of community resources:
- Develop and maintain a working knowledge of community-based resources that are available to patients, including health insurance information, support groups, detox centers, discount medication resources, dual diagnosis treatment centers, housing services, food access, etc.
- Collaborate with outside agencies and clinics in the community who are starting suboxone programs to share best practice models/techniques.
- Advocate for patient needs within the University of Minnesota and UM-Physicians organizations, as well as the broader community.
- Regularly attend webinars and other trainings related to substance use disorders and new treatment options.
- Maintain a calendar of community events and forms, participate in recovery fairs. Update existing and create new information materials to share with partners for distribution.
(5%) Harm reduction and wound care kits:
- Provide Narcan education and training to patients and/or family members, as needed.
- Keep current with evidence-based practices and harm reduction strategies to be able to provide patient-centered, culturally responsive care.
- Create, stock, and distribute Narcan reversal kits and Fentanyl and Xylazine testing kits, complete with use education and information for local resources.
- Distribute wound care kits (i.e., for injection site infection) to community partners working directly with clients with SUD.
Anticipated Starting Salary: $28.36hr to $30.76hr depending upon qualifications and experience.
Qualifications
All required qualifications must be documented on application materials. Required Qualifications:
- BA/BS plus at least four years of experience or a combination of related education and work experience to equal at least eight years.
- Interest in serving patients with opioid use disorder or other substance use disorders.
Preferred Qualifications:
- Masters in Addiction Counseling, Social Work, or related field, or Licensed Alcohol and Drug Counselor (LADC) with 2 years' experience.
- Knowledge and interest in serving patients with mental and chemical health needs.
- Experience in managing clinical care using an electronic health record.
- Knowledge of community resources and/or social service agencies.
- Experience in providing care coordination to a complex, diverse population.
About the Department
The Department of Family Medicine and Community Health at the University of Minnesota was created more than 50 years ago in response to a legislative mandate to train primary care physicians.
