Position Overview This position also develops a professional trust with the suboxone providers, assists patients in connecting with both internal and external care team providers (CPS, probation officers, treatment centers, etc.) to ensure patients are working on meeting their sobriety goals. This position requires knowledge of harm reduction strategies and available resources to patients on their road to recovery. Community Outreach and Mobile Care: This position will also participate in community outreach activities-with CUHCC staff, UMN Mobile Health initiative and other organizations to promote CUHCC's opioid-related service and connect community members to care. This position reports to The SUD Program Manager. This position works in person at CUHCC with occasional participation at community events as directed by the supervisor. Generally speaking, main working hours are Mon-Fri 8:30 AM- 5:00 PM with occasional evening and/or weekend hours. The pay for this position is $28.00/hr to $32.00/hr ($58,240.00 to $66,560 annually). CUHCC serves nearly 11,000 patients a year through over 55,000 visits annually. The patient population comes from over 12 different racial and ethnic groups that span five continents.
