Job Overview:

* This position is responsible for working independently within a team framework, while assessing the clinical, environmental, and psychosocial needs of patients diagnosed with cancer and blood disorders within the ambulatory setting. They are part of a multidisciplinary team that must collaborate for purposes of psychosocial needs that arise through course of diagnostics, treatment and survivorship. Collaborates with care teams, including but not limited to hematology, medical oncology, surgical, radiation and neurological.

* Demonstrate competency of distress, as determined by NCCN Guidelines and the Distress Screen Tool. Clinician must be skilled in basic biological assessment to determine if there is a need to notify physician (PCP or specialist) of medical changes

* Assessment of needs include; financial, medical, social, emotional and familial. Utilize clinical treatment skills to evaluate and coordinate referrals to behavioral health.

* Provide support, education, and assistance to ensure patients' and their families' needs are identified and assist to eliminate barriers to care. Responsible for keeping up to date on eligibility for current resources/services in the community. Collaborate with insurance payors, and vendors on behalf of the patient and family, and advocate on their behalf.

* Explore the emotional and social impact of illness and treatment. Utilize clinical judgement to provide emotional and bereavement support to patient and family.

Job Requirements:

Master's Degree in Social Work, from accredited university/college

Licensed Social Worker

Knowledge of hospital systems, and disease specific signs and symptoms for patients to be followed

Knowledge of community resources, especially those serving patients population both TriHealth and others

2-3 years experience Clinical Healthcare

Job Responsibilities:
