Spring Mountain Sahara is currently seeking a dynamic professional to join our team as a Discharge Planner. The Discharge Planner serves as a member of multi-disciplinary team supporting the hospital's treatment program, and philosophy and assure the deliverance of quality treatment to patients and their family members. Provides discharge planning & case management, and psycho-educational groups to age-specific populations. Stays informed of current/new community resources for discharge planning. Conduct discharge planning groups as indicated.

Spring Mountain Behavioral Services has provided mental health services to the Las Vegas community since 2001, treating children, teens, adults and senior adults with behavioral health or addiction issues. We offer a supportive and healthy environment with 24-hour care that allows people of all ages to make positive changes in their lives. Our primary location, Spring Mountain Treatment Center, focuses on the stabilization of acute symptoms for patients age 5 to 65, along with select outpatient care programs for teens. Our secondary location, Spring Mountain Sahara, houses our Adult Outpatient and Senior Adult Inpatient programs.

Spring Mountain Treatment Center is a 110-bed mental health facility that provides acute crisis stabilization services for adults, teens, and children in a secure inpatient setting. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. We provide a caring and healthy environment that gives individuals a chance to make positive changes.

Qualifications

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Develops implements and documents a discharge plan established with the patient per CMS & Joint Commission standards and with attention focused on progressing toward discharge.
Demonstrates current knowledge about full range of discharge options/community resources and utilizes them in the patient's discharge plan.
Serves as resource to patients and makes referrals to appropriate resources.
Following each patient/family contact to address discharge planning progress, a social service progress note will be entered outlining services provided.
Provides psycho educational or discharge groups as assigned. Following each group, documentation is made in the patients chart or on a group note.
May attend on/off campus meetings related to patient, social work and community issues.
Attend Treatment Team Meetings as determined by the Director of Clinical Services.
Attends educational in-services to continually update knowledge base related to behavioral health and social work issues.
Understands and complies with the intent of the Performance Improvement Purpose and Procedures.
Supports internal and external customer service standards
