In this role a Lifestyle Coach provides behavioral change methods through telephonic and chat coaching. The population group at Level2 consists of members diagnosed with Type2 diabetes who are desiring to reverse the effects of diabetes through the use of CGMs (Continuous Glucose Monitors), specialized apps and other resources. The Coaches are responsible for collaborating with a Care Team composed of Level2 Providers, Nurses, Registered Dietitians and other support areas.

Level2 is a Care Team focused on areas such as nutrition, physical activity, mindfulness and medication remediation with its members. The coach will personalize the program to the consumer to activate outcomes progression. The Coach will meet with members based on member need and risk related priority levels. The Coach will review such things as CGM data, exercise data, nutrition information, etc. in order to support the member in to moderate their glucose levels. The Coach will support each member in areas of behavioral change to ensure the member has the tools and information through technology to achieve their goals.

Primary Responsibilities:



  • Virtually complete 15-30 minute telephonic/chat coaching interactions
  • Primary focus on telephonic engagement
  • Behavioral change management of diverse members (with Type 2 diabetes)
  • Ability to empathize, motivate, and encourage all members towards healthy changes

Required Qualifications:



  • National Board Certified Health and Wellness Coach (NBC-HWC) or other Coaching Certification
  • 1+ years or Coaching/Behavior Change experience with proven results
  • Experience working with groups and individuals virtually (telephonic, etc.)
  • Experience working in healthcare environment regulated by HIPAA
  • Proficient in Microsoft Word, Excel and PowerPoint
  • Bilingual Spanish
  • Proven solid written and verbal communications skills


Preferred Qualifications:



  • Additional professional certifications in health/wellness
  • Experience in using Sales Force, telephony systems and CRM software

Residents Only: The hourly range for this role is $23.22 to $45.43 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
