Assesses patient needs based upon a face-to-face or telehealth clinical interview, case history review and information obtained from collateral sources such as family members, the referent or other parties. Recognizes signs/symptoms associated with risk of harm to self and/or others and implements appropriate interventions as necessary. Develops and implements an appropriate treatment plan of care for the patient based upon relevant clinical information and diagnosis. Consults with family members or professionals from other agencies/organizations as necessary in developing optimal treatment services.
