Gundersen Health System is seeking a RN, Care Coordinator for our Hospice department. This is your opportunity to work primarily day shift while doing the work you love in the beautiful coulee region.

What you will do:

  • 0.9 FTE=72 hours/every two weeks (salaried)
  • Work primarily daytime hours, rotating every 3rd weekend and holiday rotation
  • Occasional on-call coverage for evenings and nights
  • Provide compassionate care to families and patients thru end-of-life transitions within a 50 mile radius of La Crosse, WI
  • Work with a multidisciplinary team consisting of nurses, social workers, chaplains, bereavement coordinators and hospice aides
What you need:
  • Associate Degree in Nursing, bachelor's degree in nursing needed within 6 years
  • Minimum of 1 year RN experience
  • RN licensure to practice in the state of Wisconsin upon hire
  • RN licensure to practice in the state of Minnesota within 3 months of hire
