Come join our team as a Direct Care Associate at Mississippi's largest provider for adult mental healthcare. This position will be located in Pearl, Ms.

ESSENTIAL JOB FUNCTIONS:



  1. Provide assistance with activities of daily living, i.e., eating, meal planning, bathing, dressing, hygiene/toileting, etc.
  2. Provide assistance with light housekeeping needs that are directly related to the health and well-being of the eligible individual, i.e., bed-making, dusting, vacuuming, etc.
  3. Provide assistance with accessing community resources/shopping/appointments
  4. Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner
  5. Adhere to safe transportation procedures at all times
  6. Follow appropriate protocol for completion of I/A and Serious Incident reports
  7. Work flexible schedule based on the family's needs and to foster maximum use of authorized hours
  8. Attend trainings/in-services as requested and adhere to requests for personnel update information in a timely manner
  9. Adhere to UltiPro usage policies, procedures, and timelines
  10. Adhere to Activity Plan and/or family instructions regarding personal care of the eligible individual
  11. Keep supervisor abreast of pertinent information relevant to the provision of services
  12. Maintain a positive attitude and rapport with the eligible individual and family
  13. Adhere to confidentiality policies and procedures
  14. Refrain from excessive and unwarranted absenteeism and/or tardiness
  15. Adhere to all Millcreek and HCBS waiver program policies and procedures.
  16. Follows all safety policies and adheres to all worker's compensation program guidelines.
  17. Other duties as assigned.
