CLINICIAN

FULL TIME - EXEMPT

General Summary:

The Rhode Island Systems, Engagement and Navigation (RISEN) Masters' Level Clinician for the Housing First Program will provide supervision to the RISEN Case Manager who is embedded in local housing authorities (PHA) and/or shelters to identify individuals that have mental health or substance use needs and provide linkages to address social determinants of health. The goal of RISEN is to reduce the number of people experiencing homelessness in Rhode Island due to untreated mental illness, substance use or co-occurring disorders through connection to treatment and services.

Essential Responsibilities:




  • Provide supervision and support to the RISEN Case Manager
  • Build relationships with local agencies and providers to ensure a smooth referral process for those identified for RISEN enrollment
  • Collaborate with Case Manager and local PHAs/shelters to develop on-site schedule and build rapport with residents and housing staff
  • Attend and participate in RISEN advisory committee
  • Assistant Program Manager in monitoring goals outlined in the RISEN program
  • Screen individuals identified as homeless or at risk for eviction for social determinants of health, behavioral health needs, housing needs and provide linkages to appropriate behavioral health, substance use, and housing services
  • Participate in care coordination or service delivery planning for enrollees that focuses on stability across services and housing transitions
  • Ensure that warm handoffs are made to community providers based on participants' willingness to engage in treatment
  • Ability to identify social determinants of health and coordinate appropriate referrals as needed (i.e. state and local benefits such as Medicaid, Supplemental Nutrition Assistance Program (SNAP), disability benefits, veterans' benefits, etc.
  • Assist clients in developing and implementing a housing plan to retain permanent housing as soon as possible after financial assistance ends
  • Collaborate with other service providers through case conferencing as needed in support of our common mission to provide referrals for individuals who have been identified as in need of services
  • Coordinate with crisis services (BH Link) and 988 to connect individuals in crisis who are experiencing homelessness or at risk of homelessness, if needed
  • Provide assessment evaluations, short-term counseling and crisis intervention as needed
  • Monitor and evaluate program participant progress through internal tracking system
  • Coordinate with HMIS Agency Manager to ensure all participants experiencing homelessness have been entered into the Coordinated Entry System (CES)
  • Maintain client and activity records related to all RISEN activities
  • Adhere to Housing First model utilizing motivational interviewing, trauma informed care and harm reduction practices that are culturally competent, client centered and evidence based
  • Maintain timely, accurate records, documentation, and reports as required
  • Accept other duties and responsibilities as assigned

  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.

Qualifications

Education, Experience and Competencies:



  • Master's Degree in the human service field of social work or counseling preferred.
  • Previous experience supporting families or individuals with special care needs or disabilities or families or individuals accessing housing programs and services
  • Demonstrated ability to work both independently and as an effective team member
  • Demonstrated experience working with diverse populations
  • A combination of education and experience demonstrating acquisition of the skills and abilities required



Certifications, Licenses Requirements:




  • Licensed at an independent level (LMHC, LMFT, LICSW) preferred.
  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
