About the Unit

The Telehealth RN is accountable for providing nursing care in accordance with relevant federal requirements, state laws and nurse practice acts, regulatory standards, standards of professional ambulatory care nursing practice and other relevant professional standards, and organizational policies (AAACN, 2011; ANA, 2015a). Delivers high quality nursing services per system standards including: providing safe and value based dispositioning, evidence based home care advice, operating within scope of licensure and appropriate escalation to providers.

Scope of Work

Assess patient telephonically using symptom based guidelines to disposition a patient to the right location of care at the right time and with the right provider.
Manages and documents calls in a timely, efficient manner consistent with standards. Contributes to meeting service level commitments by handling calls in a manner that supports overall department goals. 80% of calls answered within 30 seconds or less.
Collaborates with office clinical staff and on call providers for most appropriate plan of care, if indicated and collaboration with clerical team members for call intake
Employs critical thinking and synthesis of objective and subjective data obtained during the assessment to identify priorities and care needs.
Utilizes a broad knowledge base of nursing and health sciences and applies clinical expertise rooted in the nursing process.
Use evidence based information to ensure patient safety and quality of care while improving patient outcomes.
Recognize signs and symptoms of an emergency and readily identify how to best support patients and families in the management of an emergency.

Qualifications
  • Preferred Bachelor's Degree Nursing

  • 5 years of relevant experience previous nursing or CDE clinical experience Required

  • Prior teaching with patients of all ages Preferred

  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
