Job Summary

Coordinates the admissions process to ensure smooth transition for patients and families.

Essential Functions
  • Coordinates the admissions process including communication to referral sources, patients and families, and teammates.
  • Fosters communication and coordination among individuals, departments, and referral sources and keeps physicians and administration informed of census and potential referral issues.
  • Provides health teaching and/or in services to referral sources and facilitates orientation to the Hospice and Palliative Care programs to promote referrals.
  • Determines the functional status relative to the functional independence measure prior to transfer and helps in deciding appropriateness of Hospice/Palliative Care potential.
  • Performs an admission screen based on appropriateness and psychological, social and financial considerations.
  • Discusses with referral sources the appropriate equipment necessary for admission.
  • Communicates medical appropriate and other special needs/considerations about patients to bed management and program team (i.e., infection control).
  • Ensures completion of logs, referrals to admission conversion, and identifies barriers.
  • Orients the patient and family to Hospice and Palliative Care.
  • Reviews the brochure and admission documentation as they pertain to the family and discuss transfer to the hospice house as well as financial/funding concerns.
Physical Requirements

Work consistently requires frequent car travel, walking, standing, sitting, some lifting, reaching, bending, stooping, pushing and pulling. Must lift some articles up to twenty- five pounds. Must have the ability to move about to accomplish tasks. Ability to clearly and concisely express and exchange ideas via spoken and written English words

conveying detailed information to others accurately and, on occasion, quickly. Must have excellent presentation skills. Travel by car required with frequent day trip and occasional overnight travel about once per month. Must have a valid North Carolina drivers license with no DUI record and a vehicle to conduct such travel.

Education, Experience and Certifications

Bachelor's Degree preferred. High school diploma or GED required. A minimum of four years experience in long-term care or related areas required. Must have an excellent working knowledge of Medicaid, Medicare and Managed Care. Requires exceptional interpersonal and communication skills.
