OverviewThe Clinical Review & Medication Therapy Management Pharmacist will be responsible for completing prior authorization reviews and assisting in formulary management, pharmacy and therapeutics committee activities, communication with physicians and pharmacists, and communication with staff on departmental policies and procedures. On call duties are a requirement of the job. They will also be responsible for completing work to support Independent Health's Medicare MTM program and pharmacy quality initiatives for our Medicare, Commercial and MediSource lines of business as needed. This will be done through interactive telephonic and/or claims review of targeted patients' prescription, medical and laboratory claims history to identify opportunities to improve therapeutic outcomes and reduce the risk of adverse events. The Pharmacist will contact identified patients to provide education and coaching as well as to identify potential opportunities to improve patient care. Additionally, they will work with prescribers to facilitate appropriate changes to medication regimens. Qualifications
