LSSI is hiring a full-timeCounselor Case Manager-Team Lead, Monday through Friday from 9am to 5pm. Provide intensive community-based management services to individuals with serious mental illness to promote community functioning and independence and to decrease psychiatric admissions, nursing home readmissions, and crisis episodes. Individuals may be part of the Colbert or Williams Consent Decrees.

Essential Functions:
  • Provide training, guidance, and support to all new Community Support Team staff.
  • Provide and coordinate a continuum of treatment services.
  • Carry a caseload of clients providing supportive counseling and concrete services assigned by Supervisor and collaborate with other internal and external programs and treatment providers in order to provide these services.
  • Ensure all documentation is completed in a timely manner and meets Rule 140/132 and/or MCO requirements.
  • Transportation of clients and community visits are required. Services will be provided in multiple locations.
  • Comply with documentation and productivity standards.
  • Adherence to ethical standards of treatment according to NASW Code of Ethics.
Position Qualifications:
  • Bachelors is required. Masters preferred.
  • LSW/LPC, LCSW/LCPC preferred.
  • 1-3 years of increasing leadership responsibility.
  • 1-3 years of experience working with mental health and/or substance use population is preferred.
  • Proficiency in EHR documentation and review.
  • Familiarity with accreditation and licensure standards.
  • Ability to work in a team environment.
  • Excellent written and verbal communication skills.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • Valid Illinois statutory and liability insurance coverage is required.
  • Valid Driver's License in good standing for the state of residency and insurance. Access to reliable transportation is required.
