Position Summary

The Medication Therapy Management (MTM) Coordinator is responsible for collating health plan data and generating outreach opportunities and maintaining general oversight of performance gap reporting under the direction of the Lead Pharmacist. The Coordinator will also support the MTM team with scheduling outreach appointments and preparing patient charts for outreach.

Essential Functions and Responsibilities of the Job

* Ability to be organized and pay attention to details and follow through. Ability to problem solve to logical conclusion and demonstrate initiative and responsibility.

* Successfully learn and navigate various computer programs And Epic/Nextgen EMRs.

* Communicate clearly and thoroughly, provide excellent customer service and be able to work both independently and as part of a team.

* Review health plan data to determine performance gaps as appropriate and determine outreach focus

* Print and mail communications to patients and/or physicians as well as assemble welcome packets for MTM and mailing them out in a timely manner.

* Answer incoming phone calls and checking MTM voicemail and direct the message to the appropriate person.

* Other related duties and projects, as assigned.

* Schedule patients as needed for consults

* Manage messages and inbound calls for MTM team

* Pull all HP data for pharmacy related measures and pull into appropriate files for analysis

* Conduct basis analysis of HP plan data under direction of team leader

* Serve as the point person to pull materials necessary for physician or patient distribution

* Work as a team player and communicates in a positive manner with co-workers, managers, and other contacts

* Other related duties and projects, as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
