The Nurse Navigator performs basic nursing skills, patient navigation, and data management in the outpatient setting, with a focus on enhancing both the patient experience and the quality of interdisciplinary medical care provided by this Korman Respiratory Institute Program of Excellence. The Nurse Navigator works in collaboration and continuous partnership with chronically ill or high-risk patients (Neuro-Oncology, Neurosurgery, and/or those with a diagnosis of a brain disease). A critical quality for success in this position is the ability to develop high-functioning interpersonal relationships with patients, physicians, and scheduling services for navigation of the Jefferson Health system.

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Serves as the point of contact, advocate, and informational resource for patients diagnosis, their care team, & family/caregiver(s).
• Promotes timely access to appropriate care such as helping to schedule appointments from first call to first treatment.
• Gathers records, organizes and supports care team.
• Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, specialist and family/caregiver(s)
• Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patient's electronic record
• Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs
• Identifies patient health illness and psychological history related to clinical trials/clinical research
• Participates and helps coordinate weekly tumor board
• Helps develop educational materials for patients, families and other staff.
• Plans, develops, and facilitates existing support group.
