Related positions are contingent on board approval. Coordinates the development, integration and implementation of activities associated with the transitioning of students with educational disabilities to appropriate post-secondary opportunities. Works collaboratively with school-based personnel, outside agency service providers and colleges and universities to provide a holistic approach to the transition process.
Essential Functions
- Coordinates, manages and oversee the daily operations of City Schools age appropriate transition programs for students with disabilities.
- Prepares and maintains documentation of programs and activities.
- Works cooperatively with area special education coordinators and school-based administrators in the identification and training of school-based case managers (guidance counselors and special education teachers as designated by the IEP team).
- Plans professional development activities and provides technical assistance to school-based transition case managers, special educators and multi-disciplinary teams on the transition process.
- Works cooperatively with area special education coordinators, principals, IEP Instructional Associates, the Department of Curriculum and Instruction, and Office of Career and Technology Education to ensure that the transition IEP is developed using appropriate IEP procedures, and that it reflects the career / occupational competencies required to access adult services agencies and the competencies required to participate in and contribute to the community.
- Participates in the development of transition training packets and the implementation of transition training sessions for students, parents, multidisciplinary team members, and other designated school-based personnel.
- Assists in the development of coordinated interagency strategies to collect, share, and use student assessment data, with appropriate input and authorization of students and families.
- Assists in the development of appropriate individual guidance plans and Career and Educational Planning Portfolios for students with disabilities; and with transition case managers to incorporate information from the individual guidance plan and portions of the IEP.
- Develops and implements methods and strategies for increasing families' knowledge and skills about transition-related issues and topics, including transition-focused educational program development.
- Identifies family service needs related to transition outcomes and assists families to connect with support networks.
- Identifies post-school service needs using transition planning documents in conjunction with relevant agencies to facilitate linkages to adult services.
- Provides supervision and guidance to transition team.
- Functions as project manager for corrective actions plans for transition.
- Performs and promotes all activities in compliance with equal employment and nondiscrimination policies; follows federal laws, state laws, school boards policies and the professional standards.

Desired Qualifications
- Master's degree in education, vocational rehabilitation, transitioning, guidance or closely related field.
- Valid Maryland State Department of Education Advanced Professional Certificate.
- Three years of teaching and/or related services experience working with students with special needs.
- Knowledge of best practice in transition services for students with disabilities.
- Knowledge of transition planning and anticipated services in Individual Education Plan (IEP) programs.
- Knowledge and experience with federal and state legislation guidelines governing the IEP process.
- Interpersonal skills that ensure effective team building.
- Organizational, management, leadership and supervisory skills.
- Excellent verbal and written communication skills, including the ability to make persuasive presentations.
- Proficient in the knowledge and use of technical computer applications.
- Ability to plan and deliver training and professional development for transition services and activities.
