Qualifications

  • Certified Community Health Worker Specialist I, II, or III
  • Three (3) years of experience interacting with community members or in community health/public health services.
  • Must be able to speak language of the population served.

Skills

  • Skill in use of computers and related software applications, including e-mail.
  • Ability to communicate and set boundaries.
  • Knowledge of barriers to care that may be encountered by patient.
  • Knowledge of community agencies and resources.
  • Skill in time management.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of stakeholders in a diverse community.

Working Conditions

  • Comfortability working in an Emergency Department and/or clinical setting, including Behavioral Health.
  • Can work in both clinical and community-based settings, including client's homes.


Responsibilities

  • Lead training and shadowing opportunities for new team members, ensuring a smooth onboarding process.

  • Conduct specialized classes related to Community Health Worker (CHW) expertise and implement CHW Core Competency training programs.

  • Assist in recruiting, selecting, and hiring new staff members.

  • Represent Presbyterian at local, regional, and state CHW events/meetings to stay connected with industry trends and best practices.

  • Patient Support & Advocacy:

    • Screen patients for social determinants of health and connect them with community resources.

    • Utilize motivational interviewing and other techniques to engage patients and families in self-management.

    • Act as a patient advocate, ensuring their needs are met and voices are heard.

    • Educate patients, families, providers, and care teams on social determinants of health and available support services.

  • Stakeholder Collaboration:

    • Build strong relationships with internal and external stakeholders, including CHWs and Peers, to improve community health outcomes.

    • Continuously expand knowledge of community resources and services to better assist patients.

    • Facilitate patient access to community resources, including social service agencies and application assistance.

    • Initiate closed-loop referrals for programs patients may be eligible for.

  • Teamwork & Relationship Building:

    • Work collaboratively within a multidisciplinary team, fostering a positive and supportive environment.

    • Establish and maintain strong working relationships with clients, providers, case managers, social workers, agency representatives, and office staff from diverse backgrounds.

    • Help reduce cultural and socio-economic barriers between patients and healthcare institutions.

  • Additional Duties: Perform other responsibilities as needed to support the team and community.
