Responsibilities
  • Provide Home-Based Pediatric Physical Therapy - Conduct comprehensive assessments, develop individualized treatment plans, and deliver skilled therapy interventions to pediatric patients in their homes. Ensure high-quality, evidence-based care to improve patient outcomes and enhance independence.

  • Coordinate Patient Care & Education - Collaborate with physicians, caregivers, and interdisciplinary team members to develop and update care plans. Educate patients and families on treatment goals, equipment use, and emergency preparedness to support ongoing therapy and recovery.

  • Maintain Documentation & Compliance - Complete accurate and timely documentation in accordance with home health regulations, accreditation standards, and agency policies. Participate in quality assurance activities, peer reviews, and maintain professional competency in pediatric home health care.


Qualifications

Education:

  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association
  • Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.

Experience:

  • One year experience caring for similar patient population as to be assigned.

License(s)/Certification(s):

  • Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
