We are seeking aBlended Case Managerto join our Adult Behavioral Health team in ourSharon Hill, PAlocation. Earn $21.83 per hour with benefits for a 37.5 hour week, or $23.83 per hour with the selection of enhanced pay and a scaled back benefits package. Position Details The Blended Case Manager will provide resource coordination, recovery services, and symptom management using a strengths-based model of case management to 20-25 Adults living with a mental illness. The BCM will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence, and chart documentation. The ideal candidate will possessoneof the following: A high school diploma plus a minimum of 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology, or other social science required and two (2) years of behavioral health direct care experience required;or A Bachelor's degree with major course work in sociology, social work, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, healthcare administration, counseling, or education. The ideal candidate will havebothof the following: Must possess a valid driver's license, insurance and have a clean driving record. Own a reliable vehicle.
