The Population Health Quality Coordinator, under the direction of the Quality Coordinator RN/Quality Improvement Analyst and Market Leadership is responsible for leading the quality improvement activities needed to perform favorably on contracts and ACO financial incentives as well as to deliver overall high-quality care to appropriate patient populations. The Population Health Quality Coordinator will work closely with primary care providers and clinical staff to achieve key measures of success, closing gaps in care and accurately coding patients' severity of illness in their documentation. This position participates in collaborative meetings and process improvements involving ACOs, insurance companies' quality program managers and the primary care practices.

Job Duties/Responsibilities:

  • Participates in decision-making and process improvement and communicates effectively within and beyond assigned team.
  • Focuses on the problem or issue, not the person.
  • Assists with daily clerical operations, including making copies of documents as necessary.
  • Schedules patient appointments as necessary, such as return visits and Specialists.
  • Treats patients and families with respect and dignity, and advocates for their care and choices on their behalf.
  • Other duties as assigned.

Qualifications

Requirements:

  • High School Diploma or equivalent required
  • Technical School/1 Year College preferred
  • Medical Assistant certificate required
  • Current BLS/CPR certification required
  • Minimum of 3 years' experience as a Medical Assistant
  • Fluent with medical terminology
