Unique home visiting based opportunity to service clients in the community. APP will travel between 6-30 miles daily to visit clients.



  • Conducting thorough physical examinations.
  • Obtaining detailed medical, social, and family histories.

GENERAL SUMMARY: Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience.

EDUCATION/EXPERIENCE REQUIRED:



  • Master's degree in nursing -OR- requires a bachelor's degree, completion of an AMA-approved Physician Assistant program, Licensure as a Physician Assistant
  • 1 year APP experience required


CERTIFICATIONS/LICENSURES REQUIRED:



  • Certified Nurse Practitioner. Licensed as a Registered Nurse or Licensure as a Physician Assistant
  • Current BLS or ACLS as required.
  • Current ATLS/PALS provider status as required by department.
