DESCRIPTION OF WORK

In this important role, you will evaluate consumers' needs and levels of care by completing written assessments of consumers' physical health, cognitive functioning, activities of daily living, nutritional risk, informal supports, physical environment, and financial concerns. This information will be used to develop a care plan with consumers designating services and support to meet their unique needs and to assist them in obtaining services based on their needs. Responsibilities will include maintaining a caseload of consumers receiving services and support through the Agency, completing ongoing case documentation for each consumer, and providing information about and referring consumers to appropriate social service organizations. You will have the opportunity to provide care management to aging consumers and their informal support through ongoing communications to include telephone calls, home visits, reassessments, and referrals. Additional duties to include but not limited to:
- Working in close cooperation with other social agencies, hospitals, clinics, courts, and community resources in planning to meet the needs of the consumers and assist the consumer in utilizing these resources
- Assisting aging consumers with the completion of forms and applications, such as PACE, Rent/Tax Rebate, Social Security, Black Lung, and Department of Military and Veterans Affairs documents
- Providing supportive services for consumers who are in situations detrimental to their well-being

Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
- Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Northumberland County. If no eligible candidates who live within Northumberland County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
