Your goals and objectives include:

• Develop initial rapport with patients and their families at time of admission. Maintain regular communication with family members as appropriate
• Assist patients in identifying and implementing appropriate discharge and aftercare plans.
• Provide different interventions, according to training, expertise and scope of practice, with the purpose to help therapeutic movement toward treatment goals.
• Contact aftercare professionals/treatment facilities to provide patient information that will aide in a smooth transition of care, OR coordinate aftercare appointments
• Obtain Release of Information Forms for outside resources.

Your Qualifications and Requirements will include:

• Bachelor's degree REQUIRED; Bachelor's degree in Social Work preferred
• Clerical and organizational experience is required
• Excellent written and oral communication skills, time management, and social skills
• 21 years of age or older preferred
• Ability to pass a pre-employment background check and drug screen
