JOB DESCRIPTION: The Dialysis Social Work Case Manager (SWCM) is an integral member of the interdisciplinary care team in the Acute Dialysis Unit located inside UVM Medical Center in Burlington. This position will primarily support patients as they initiate dialysis and transition to outpatient dialysis services outside of the hospital. This role emphasizes patient-centered care, prevention, health promotion, care coordination, and chronic disease management. The SWCM conducts comprehensive psychosocial assessments and provides targeted interventions to address barriers impacting patients' treatment adherence, health outcomes, or successful transitions to outpatient care. Utilizing clinical social work and case management expertise, the SWCM offers therapeutic support, advocacy, resource linkage, and practical assistance to patients and families throughout their dialysis journey. Collaboration with hospital-based interdisciplinary teams as well as outpatient dialysis teams is essential, ensuring seamless patient transitions and continuity of care. This position uniquely blends clinical social work and case management, guiding patients and families in navigating healthcare systems, understanding dialysis treatment options, and accessing essential community-based resources. EDUCATION: MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred. EXPERIENCE: Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required. |
