DETAILED JOB DESCRIPTION

Collaborate with the healthcare team to provide patient care, adhering to American Nurses Association (ANA) Scope and Standards of Practice, Code of Ethics, and regulatory requirements. Participate in interdisciplinary care planning, coordination, and evaluation, including patient/family education and discharge planning.

Act as a resource for patients, families, nursing staff, and healthcare professionals in the area of expertise.

Manage continuum of care for transplant patients, including consultation with physicians, organ procurement organizations, and transplant team. Maintain communication with patient's dialysis unit (if applicable) and referring physician.

Meet with transplant candidates in clinic to provide education on procedures, frailty assessments, and necessary paperwork; offers ongoing education throughout transplant process.

Assess, plan, implement, and evaluate patient care for assigned populations. Provide direct patient care as required. Perform triage/symptom management and routes patients to appropriate care levels. Ensure care pathways/order sets are followed.

Request and review outside medical records for transplant evaluation.

Prepare patient letters and redacted profiles for medical review and regulatory purposes.

Schedule testing for new transplant candidates and coordinate annual testing for waitlisted patients. Maintain appropriate monitoring of patient status throughout work-up and while on the deceased donor organ transplant waiting list.

Arrange living donor surgeries and coordinate scheduling logistics. Manage all transplant procedure scheduling, ensuring timely and accurate bookings. Assist in the evaluation and selection of potential living donors.

Maintain candidate data within UNET. Review histocompatibility results for patients and transplant pairs. Submit required documentation to United Network of Organ Sharing (UNOS) to maintain patient status.

Lead weekly team meetings to review patient cases and progress. Maintain communication with dialysis units and referring physicians, ensuring ongoing collaboration.

On-call availability for organ offers, patient consultations, and surgery scheduling.

Oversee crossmatch ordering and track organ deliveries to the transplant center.

Maintain accurate documentation of all patient interactions, consultations, and follow-up care.

Promote patient satisfaction and resolve concerns related to healthcare services.

Participate in the development and implementation of performance improvement activities.

May complete unit/department-specific duties as outlined in department documents.

REQUIRED QUALIFICATIONS

Bachelor of Science in Nursing degree or an equivalent combination of education and experience from which comparable knowledge, skills and abilities can be acquired.

Two (2) years of RN experience in an area of specialization.

Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days.

PREFERRED QUALIFICATIONS

Additional license/certification requirements as determined by the hiring department.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. This position is very active and requires ambulating, reaching, and moving objects weighing up to and over 20 lbs.

This role also routinely comes into contact with patients who may have contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.
