Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. The HDSC Patient Care Coordinator works within a multi-disciplinary team providing integrated services to patients seen at Hooper. This individual supports Hooper Inpatient and Outpatient services and programming, including ongoing Medication-Supported Recovery (MSR) services offered through the Hooper Bridge Clinic. The Care Coordinator is responsible for providing administrative support to the treatment team, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient centered. A key element of this role is the assessment of patient care-coordination needs and serving to identify and solve for solutions regarding barriers to clinical engagement within the continuum of care. This role has access to mental health billing and service records. Location: Hooper Detox Stabilization Center (1535 N Williams Ave. Portland, OR 97227) Schedule: Monday-Friday 7:30am - 4:00pm Compensation: Starting at $24.52/hr MINIMUM QUALIFICATIONS:
- High School diploma or GED required. Prefer Bachelor's Degree or higher in health or social service related field of study.
- At least two years experience in human services, or social services in a related setting.
- Must have current CPR certification or must be able to obtain in one month.
- Has the competencies necessary to:
- Communicate effectively orally and in writing;
- Understand healthcare related terminology;
- Provide outreach, engagement and supportive services
- Bi-lingual speaker of Spanish preferred.

- Ability to independently track information pursuant to individual clients and their navigation through the continuum of care.
- Must pass a pre-employment drug screen, TB test, and background check.
- Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Manage inpatient and outpatient referrals for services.
- Initiate prior authorizations for medication through insurance companies.
- Coordinate with IT and Quality Management to implement and maintain treatment program data.
- Track and report data related to patient program admission, progress, discharge, transition and other criteria as needed.
- Coordinate Utilization Management activities.
- Support care team and clients by participating in triage and evaluation of client needs.
- Authorization for Care: Check benefits and collect and submit clinical information necessary to submit prior authorizations and continued stay authorizations for care.
- Care Coordination and Communication: Coordinate services with all members of the care team and the client's support network. Work collaboratively with pharmacies, insurance companies, medical providers, and behavioral health and primary care teams both internally within CCC and externally with community providers. Assist with patient care by problem-solving, eliminating unnecessary barriers to accessing services, scheduling appointments, checking Pre-Manage, and checking the state prescription drug monitoring system.
- Documentation: Care Coordinators document Care Plans in EHR for all patients.
- Work within scope of training and experience and seek consultation when appropriate.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
- Attend all mandatory CCC trainings in a timely manner.
- Perform other duties as assigned.
