RN Population Health Program Manager

M-F, Days

Location: Remote/work from home with strong preference for candidates residing within the UPH geographies of Iowa, Illinois, & Wisconsin.

This role is responsible for the identification, development, implementation, and evaluation of projects supporting UnityPoint Health's population health and chronic disease management strategies and business plans across the continuum of care.

  • Partners with physicians, cross-continuum leaders, and front-line care givers to achieve the

Triple Aim in the following areas: health promotion, chronic disease prevention & management, and social determinants of health. Program development, deployment, and team alignment.

  • Leverage analytics for program development and growth
  • Optimize EHR workflows in support of an engaged and productive workforce
  • Maintain and establish new community connections to improve the healthcare ecosystem
  • Foster and promote connectivity between healthcare providers across the system continuum and across disease states
  • Bring awareness of national trends and best practices to inform improvement work

Responsibilities

Leadership

* Builds trusting relationships to advance core business strategy.

* Allows for two-way conversation between associates and leadership.

* Engages team members in shared decision-making and promotes patient-centered decisions.

* Leads and participates in a multidisciplinary team.

* Serves as a change agent, assisting others in understanding the importance, necessity, impact

and process of change. Facilitating change cycle while engaging teams to participate in driving

change that produces results.

* Notifies leadership of concerns or dynamics that have the potential to escalate or negatively impact

patient or team member quality, safety, or experience.

* Serves as a professional role model and mentor to future nursing leaders/healthcare professionals.

* May be requested to travel to conferences and meetings.

* Some travel will be required to support regional project deployment

Planning/Development

* Identify future organizational needs to ensure program remains competitive, grounded in evidence, team-driven, and sustainable.

* Collaborates with leadership to facilitate planning, implementation, and evaluation of strategic

plans and initiatives.

* Guides priorities and focus of work to achieve program goals.

* Links clinic, inpatient, post-acute and outpatient services to promote coordination of care across the continuum to promote operational efficiency and system connectivity across all participating regions.

* Always considers patient safety in the development of clinical systems, processes, policies, and procedures.

* Collaborates with senior leadership to support program/practice changes to continually improve resource utilization.

Quality Improvement & Compliance

* Ensures that written organizational clinical policies and procedures are reviewed and updated in

accordance with evidence-based practice standards.

* Facilitates quality improvement activities and processes.

* Measures and analyzes performance from various perspectives (i.e., customer service, staff

development, program growth, business planning, and fiscal responsibility).

* Collaborates with team members to assess current environment and implement evidence-based

practice and process improvement measures to advance program goals.

* Reflects organizational and departmental goals in the performance improvement plan, which

includes shared process improvement and outcome measures. Ensure outcomes are evidence-

based when applicable.


Qualifications

Education:

* Completion of an accredited nursing program

* Bachelor's Degree required

Experience:

* Five to seven years of clinical experience required

* Two to three years of leadership experience preferred, with evidence of successful change management

Licensure/Certifications:

* Current RN licensure in the State of Iowa or compact state

* Valid driver's license when driving any vehicle for work-related reasons

Knowledge/Skills/Abilities:

* Knowledgeable about patient care and nursing practice across service lines and management principles and practices including performance management and performance improvement

* Knowledge of the current healthcare environment and its impact on nursing practice
