Position: Social Worker

Location: Johns Hopkins Bayview Medical Center

Summary

Join a dynamic healthcare team where your expertise as a Social Worker makes a real difference. As a Social Worker II, you'll play a vital role in supporting patients and families through some of life's most critical moments. Working collaboratively with physicians, nurses, and interdisciplinary team members, you'll assess psychosocial needs, address barriers to care, and guide families through the healthcare journey with compassion, advocacy, and expert guidance.

Your work will directly impact discharge planning, crisis intervention, family education, and the overall patient experience - helping ensure the best possible outcomes for every individual in your care.



Key Responsibilities



  • Serve as a clinical expert in your specialty area, offering mentorship, consultation, and guidance to fellow social workers and care team members.



  • Actively participate in research, grant writing, academic presentations, or publications as applicable.



  • Provide coverage during staff absences, vacancies, and on-call periods.



  • Conduct initial psychosocial screenings for all assigned patients and complete comprehensive assessments where indicated.



  • Develop and implement individualized care plans in partnership with patients, families, and care teams.



  • Deliver direct psychosocial support, including counseling, crisis intervention, and discharge planning.



  • Coordinate closely with interdisciplinary teams to ensure seamless transitions and continuity of care.



  • Maintain thorough and timely documentation in alignment with departmental and regulatory standards.



  • Leverage a strong network of community and regional resources to support patient needs.



  • Assist with referrals, capacity determinations, legal processes (e.g., guardianship), mental health and abuse assessments, and end-of-life care planning.



  • Facilitate and lead family meetings to align care plans and support decision-making.

Required Credentials



  • Licensure: Active LCSW-C or LMSW (required)



  • Additional training or certifications in crisis intervention, trauma-informed care, or care coordination are a plus.




Salary Range: Minimum $34.11/hour - Maximum $56.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
