Responsibilities

Are you ready to make a difference in the lives of individuals with psychiatric needs? As a Case Manager, you'll be the go-to person for providing support and services to our amazing patients! Whether in person or remotely, you'll be based at our locations, helping people on their journey to better mental health.

Here's what you'll do:

  • Welcome New Faces: Help facilitate admissions and get patients started on their treatment journey.
  • Lead the Charge: Run engaging psychoeducation groups where you'll share valuable information and tips.
  • Team Player: Work closely with doctors, therapists, and other team members to create and adjust treatment plans that fit each patient's needs.
  • Stay Connected: Keep in touch with patients and their support systems, making sure everyone is involved in treatment decisions.
  • Share Your Knowledge: Educate patients and their families about everything from the admission process to safety plans and patient rights.
  • Plan for Success: Develop and revise treatment and discharge plans, keeping everyone in the loop about what's next.
  • Be a Supportive Advocate: Provide care that is patient-centered, culturally appropriate, and age-appropriate.
  • Stay Sharp: Keep your skills fresh in de-escalation techniques and trauma-informed care to help patients feel safe and supported.

Qualifications

  • Bachelor's in Psychology, Sociology, Human Services, or a closely related field required.
  • Masters degree in counseling, marriage and family therapy, psychology, human services or a closely related field preferred.
  • Experience in healthcare setting preferred
  • Experience using electronic health records preferred
  • Knowledge of Microsoft Office Products including Word and Outlook
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Following de-escalation training, ability to respond appropriately to patients experiencing a crisis

Pay range for this position is $25-30 an hour, depending on education and experience.

We are committed to providing competitive and equitable compensation. An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions. Additionally, we consider internal equity to ensure fairness within our organization.
