Responsibilities

  1. Ensures staff is educated and follow federal/state/other third-party payers' regulations regarding senior/client care. Review procedures to validate clinical staff practice are supported by the procedures. Conducts shared visits with staff ensuring competence and customer service.
  2. Organizes, plans, and supervises the orientation of all new staff, delegating specific tasks to key TEAMembers and partners with preceptors to ensure a thorough, effective orientation that meets the required milestones.
  3. Collaborates with scheduling to ensure clinical staff resources and assignments are consistent with patient care needs and productivity expectations.
  4. Collaborates with the Quality Assurance team to sustain standards of care and practice which are consistent with current trends in the industry.
  5. Leads coordination efforts on complex cases with the clinical staff to ensure the development and implementation of updates to the individual Senior/client needs are continually assessed to the individualized plan of care provided. Coordinates the episode of care with the clinical team; ensures appropriate numbers of visits for HHRG score.
  6. Provides support to the branch offices by visiting monthly (at a minimum), providing direction to the office staff, and holding case conferences on Seniors/clients in the region.
  7. Takes administrative on call as needed. May perform home visits in time of high census.
  8. Supervise, manage and establish good relationships with direct reports. Foster TEAMember satisfaction through effective and consistent leadership by listening to TEAMember concerns and taking appropriate action. Identify and remove barriers to achieve employee success.
  9. Willingly travels between Grand Island, York and other surrounding areas. Will be given a Tabitha branded vehicle for all work travel.


Qualifications

COMMUNICATION: Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job.
WORKSITE CONDITIONS: Works indoors in a facility maintained by Tabitha that will typically be in good repair, clean, well ventilated and well lit. Work is frequently performed in a standard office setting. Occasionally works in Senior/client homes which will vary significantly as far as their state of repair.
PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job. Ability to travel throughout Tabitha service areas with provided Tabitha vehicle. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability of 50 - pounds demonstrating strength and agility at Tabitha's discretion.
AGE REQUIREMENT: At least age 18.
EDUCATION: Successful completion of a Registered Nurse course of study required. Bachelor of Science in Nursing required.

EXPERIENCE: Three years of RN nursing required. Supervisory experience required. Education and/or nursing in home health and/or community health preferred. Knowledge of Medicare Home Health Regulations including the Outcome and Assessment Information Set (OASIS) preferred.
CREDENTIALS: RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR required. Licenses must be kept up to date at the expense of the employee. Valid Nebraska Driver's license (unless otherwise allowed by law) and a good driving record required. Must meet Tabitha's vaccination requirements.
