We are seeking a Direct Support Professional to join our team!

In Home and Community Supports: In-Home and Community Support is a direct service provided in home and community settings to assist individuals in acquiring, maintaining and improving the skills necessary to live in the community, to live more independently, and to participate meaningfully in community life. Examples: Social skills, appropriate interactions in the home and community, physical fitness plans, building ADLs skills such as cooking, cleaning, laundry, build personal hygiene skills, money management skills, assistance with scheduling and attending medical appointments, safety skills. Gives appropriate personal care assistance such as bathing, toileting, assistance with eating, etc. as they apply to each person's individual need requirements

Companion Care: This service is intended to assist the individual to participate more meaningfully in home and community life. This service may be provided in home and community settings, including the individual's competitive employment workplace. Examples: attend to the health and safety of the individuals served in the home and community.

Respite (15-minute units): Upon request from the family. Provided in home to ensure safety while family members are out of the home. Maximum is 16 hours a day.




  • Community Participation Support: CPS provides opportunities and support for community inclusion and building interest in and developing skills and potential for competitive integrated employment. CPS should result in active, valued participation in a broad range of integrated activities that build on the individual's interests, preferences, gifts, & strengths while reflecting his or her desired outcomes related to employment, community involvement & membership. Examples: Volunteering, socialization, safety awareness, money management in the community and working towards competitive employment. Facilitate the participation in a variety of community activities creating opportunities for people to make educated choices.
  • Assist people in developing the skills necessary to participate as a meaningful member of their community.



Administrative Responsibilities



  • Maintain all documentation as required by regulatory entities including but not limited to daily logs, incident reports, time sheets, and physician visit forms
  • Complete all documentation in a legible, objective, readable, and professional manner within prescribed timeframes as applicable.
  • Immediately report unusual incidents, medical emergencies, or situations that impact the health, safety, and welfare of those supported to the appropriate supervisor.
  • Complete daily logs for each shift worked. Submit to supervisor as directed.
  • Maintain effective communication with the family, program specialist, medical personal and other interdisciplinary team members and interact in a respectful and positive manner with all team members.



Individualized care



  • Assist in development of plans that promote independence in all areas.
  • Implement plans developed to teach skills that promote independence in the areas of personal hygiene, communication, social skills, safety, leisure skills, etc.
  • Assist people in maintaining their home and/or personal space.
  • Assist people in contacting proper authorities to help maintain the home.
  • Assist people identifying and accessing resources within their immediate community



Medication/Medical Management



  • Maintain familiarity with each medication administered to the person receiving services as appropriate.
  • Schedule and attend medical appointments with people when the agency is identified as the designated coordinator of medical care.
  • Assist people in following through on medical advice and orders
  • Immediately report changes in a person's health status to appropriate personal and family.
  • Report any issues with adaptive equipment to appropriate personal and family.



Perform other duties and responsibilities as assigned as it relates to the position.
