Mosaic is looking for a Licensed Practical Nurse (LPN) to join our team! In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records. Part Time Position: This will be a 24 hours p/wk. Typical schedule would be 3 days a week/some flexibility in these days - typically Monday-Friday flexibility/8 hour day Between 8-430 with 30 min lunch or 830-5pm with 30 min lunch.
