Rate of pay commensurate with years of experience at GHV We offer Medical, Dental, Life Insurance, 403(b) Retirement, Holidays, and generous PTO POSITION SUMMARY The Care Coordinator is responsible for ensuring continuity of care, coordinating activities aimed at linking individuals with serious and persistent mental illness to the service delivery system, and facilitating the process of accessing services and supports for individuals with the purpose of increasing life skills and achieving successful outcomes in chosen domains. ESSENTIAL RESPONSIBILITIES
- Participate in intake/screening process to determine individuals' care coordination service needs
- Provide on-going assessments of individuals' service needs and levels of functioning as they relate to specific life domains
- Collaborate with individual and collaterals in developing, reviewing, and modifying a Recovery Plan that identifies the services necessary to support the individuals' aspirations, goal achievements, and path to recovering meaningful life roles in the community
- Provide individual assistance in reintegrating into meaningful life roles in the community by supporting the process of choosing, acquiring, and keeping residential, vocational, educational, and/or social environments.
- Provide individual assistance in navigating the behavioral health system, including facilitating referrals to community-based services that support wellness, relapse prevention, and goal achievement. Facilitate knowledge, coordinate referrals, and support engagement with rehabilitation, treatment, employment, educational, self-help, advocacy, psychosocial, recreational, and other community-based resources.
- Provide and/or facilitate referrals to services that foster skill development and/or provide support to improve individual's ability to perform basic life skills critical to reclaiming life roles, maintaining wellness, and increasing self-sufficiency.
- Provide targeted interventions necessary to address immediate risks such as relapse, hospitalization, loss of housing, or involvement with the criminal justice system.
- Provide family support and psycho-education to assist individuals, family members, and care givers in understanding the effects of mental health disorders on life role functioning.
- Provide respite for care givers, as needed.
- Provide other care coordination services, as needed.
- Maintain case record on each individual in accordance with recognized and acceptable principles of recordkeeping. Insure all documentation is completed in a timely manner and complies with regulations and Agency policies and procedures.
- Participate in coordination, supervision and utilization review meetings.
Qualifications
REQUIRED QUALIFICATIONS Education:
- Associate's Degree in human services related field
Experience:
- Two (2) years' experience in human services or related field
WORKING CONDITIONS
- Must be able to Sit or stand for 8 hours/day
- Must be able to bend down or kneel occasionally (1-2 times/week)
- Must be able to use hands, wrists, and fingers with full range of motion
- May occasionally be required to lift no more than 25 pounds (1-2 times per month)
REGULATORY CLEARANCES REQUIRED:
- Must have a valid NYS Driver's License.
- Background checks are required.
