Apply clinical expertise and perform medication reviews, consults, and care plan development for members to prevent medication-related problems and improve care outcomes. Evaluate and analyze data and outcomes and use it to improve results of programs/campaigns. Participate in other responsibilities as assigned by the Director of Clinical Pharmacy Services. You Will Perform medication reviews to evaluate clinical appropriateness, identify misuse/abuse, and make recommendations for optimal therapy. Develop performance metrics to monitor and drive improvements in quality. Review and create care plans to improve medication management for members. Serve as a pharmacy liaison that provides training, analytical and educational support, and on-going performance feedback to medical groups, community providers and staff regarding pharmacy initiatives. Participate in interdisciplinary team meetings to discuss member cases and perform medication reviews to improve member health outcomes. Perform telephonic medication review with members to educate, identify opportunities for optimal therapy, ensure member medication adherence, and coordinate care with providers. Help create, develop, implement, and monitor clinical programs to improve quality metrics, medication or medical issues, and health equity. This includes, but is not limited to, creating written educational materials for members and providers, and developing creative strategies. Stay abreast of CMS rules, regulations, and quality metrics to ensure requirements are met with applicable programs. Ensure smooth and timely execution of clinical programs. Effectively communicate with various stakeholders internally and externally for successful collaboration and implementation of initiatives. Pharmacists License current & active required. 3-5 years of related experience .Performs work under minimal supervision .Handles complex issues and problems, and refers only the most complex issues to higher-level staff. Possesses comprehensive knowledge of subject matter .Provides leadership, coaching, and/or mentoring to a subordinate group. Bilingual English/Spanish Strongly Preferred. (Test will be administered to assess proficiency.)
