Description Lincoln HospitalProfessional - Clinical Summary Within MaineHealth's ambulatory pharmacy program, the clinical pharmacist will be a part of our growing primary care pharmacy team. In this role, the pharmacist will help provide seamless medication access and medication management for patients. This clinical pharmacist position will be focused on providing high quality, direct patient care through collaborative practice and supporting our primary care teams. In addition, the clinical pharmacist will support the transitions of care for patients. The clinical pharmacist will collaborate closely with primary care pharmacists throughout MaineHealth. Travel will be required to regional primary care practices. Some core functions include: * Work collaboratively with all healthcare professionals to provide safe and cost-effective patient care * Communicate directly with patients and their families on all aspects related to medication therapy promoting high levels of knowledge, engagement, and adherence. * Ensure appropriate and optimized medication therapy for patients with chronic disease including, but not limited to, diabetes, hypertension, and dyslipidemia. * Provide education to other healthcare professionals including precepting trainees to enhance clinical competency on topics related to medication therapy. * Partner with other healthcare professionals to develop evidence-based clinical pathways * Generate and analyze process and outcome measures for use in assessing medication-related patient outcomes. Demonstrate ongoing professional development including meeting continuing education requirements, keeping abreast of current professional literature and how it relates to clinical, financial and safety outcomes Required Minimum Knowledge, Skills, and Abilities (KSAs)
