Job Summary: The Disease Management Specialist Sr independently oversees and manages disease and preventive care registries. Works collaboratively with physicians, clinic staff and patients to coordinate care for identified patients with chronic disease. Leads the Disease Management team in efforts to meet health quality metrics, sustain standards, and improve patient care. Identifies situations and solves problems based on quality health care indicators (i.e. P4P, CMS), department organization, policies and personnel. Presents initiatives to leadership which strive to advance health care quality within the organization. Use knowledge of Information System to design reports. Acts as a preceptor and resource for other members of the Disease Management team. Demonstrates and always maintains strict confidentiality. Participates in data analysis to identify trends and make recommendations as appropriate. Performs other duties as needed.
Education and Experience: Bachelor's Degree required. Three years LVN experience in clinical/ outpatient setting required. Quality management experience preferred. Experience may be considered in lieu of Degree.
Knowledge and Skills: Clinical and/or outpatient disease management principles and practices within the LVN Scope of Practice. Essential Technical/Motor Skills: Able to keyboard 40 wpm. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Operate/troubleshoot basic office equipment required for the position. Essential Interpersonal Skills: Able to relate and communicate positively, effectively, and professionally with others; work calmly and respond courteously when under pressure; collaborate and accept direction. Essential Mental Abilities: Able to communicate effectively in English in person, in writing, and on the telephone; think critically; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. Essential Sensory Abilities: Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.
Licensures and Certifications: Current California State LVN License required. Basic Cardiopulmonary Life Support certification required within 90 days of hire.
