Responsibilities

Diamond Grove Center MYPAC is looking for a case manger to join their team in Starkville Miss. Diamond Grove Center MYPAC is located downtown in the beautiful city of Starkville, Miss.

This position will be fulltime with hours of 8:00am-5:00pm, flexibility is required for this wposition which would include some evenings and weekends, and being on call.Wraparound is an approach to individualized care planning that builds on the collective action of a committed group of family, friends, community, professional, and cross-system supports mobilizing resources and talents from a variety of sources to address the needs of children/youth with complex mental health challenges and their families.

What will the duties of the MYPAC Wraparound Facilitator conisist of?

The MYPAC Wraparound Facilitator is responsible for engaging the family and developing a strength-based relationship to identify, create and build a supportive Child and Family Team for the purpose of developing a single plan of care in accordance with high fidelity and quality wraparound practice.

- Engage the family.

- Conduct appropriate strengths and needs assessments to develop 14-day plan;

- Complete the initial Crisis Management Plan with the family within 24hrs of admission to MYPAC.

- With the family and child/youth identify formal and informal resources and supports to begin building a Child and Family Team.

- Begin developing the family story, timelines, family vision, goals, etc. to present at the first 14-day CFT meeting and update/revise at each CFT meeting thereafter.

- Take responsibility for setting up the CFT meetings, contact team members to ensure attendance, and facilitate the Child and Family Team meetings with each assigned child/family.

- Assemble the Child and Family Team.

- Educate new team members about the Wraparound process.

- Provide learning opportunities and materials/handouts/guides for the CFT to understand the wraparound process and role of the CFT with the family.

- Facilitate a Child and Family Team meeting at a minimum once a month, more frequently in the initial stages of development, and as often as needed to provide quality services.

- Provide the required wraparound services and contacts as outlined in the ISP each week with the CFT which includes the child, family, formal and informal supports and/or other collateral resources.

- Prepare in advance for all CFT meetings and provide agendas, handouts, copies of plans.

- Facilitate the development of an individualized service plan that includes a Crisis Management Plan to anticipate, prevent and manage crisis, within the Child and Family Team meeting.

- Work with the CFT to identify providers of services and other community resource to meet the individual child/youth and family needs.

- Make necessary referrals and assist CFT members in completing their assigned tasks or responsibilities.

Qualifications

MINIMUM QUALIFICATIONS

Bachelor's degree required in Social Work or a related field with two years of experience. Experience working directly with children and families and formal/informal systems required. Must be able to demonstrate applicable skills necessary for high fidelity wraparound practice. Must demonstrate strong oral and written communication skills. Computer skills are required. Must be available to work a flexible schedule to accommodate occasional evening and weekend hours to meet job responsibilities. Must be at least 21 years of age, have a valid Mississippi driver license, a good driving record, proof of legally appropriate auto liability insurance, and must be willing to use personal vehicle for agency business.

SPECIAL SKILL AND KNOWLEDGE REQUIREMENTS:

1. Must have a working knowledge of complex trauma and evidenced-based treatments.

2. Must demonstrate competence in writing program materials, completing reports and assisting with preparation of reimbursement documents related to payment for services provided.

3. Must maintain valid Driver's License and an acceptable driving record.

4. Must be eligible to qualify for CPR certification

5. Demonstrates leadership, communication, and interpersonal skills.

6. Cardiopulmonary Resuscitation (CPR) certification
