A Brief Overview

The Advanced Practice Provider is responsible for providing quality care for patients and increasing patient access and satisfaction. Standard tasks to be performed by a NP/PA as specified by the Ohio State Board of Medicine and UH entity specific Medical Bylaws.

The CAN Kidney program serves patients with advanced chronic kidney disease (CKD), addressing gaps in care and navigating the difficult transition from advanced CKD to dialysis and transplant. Additionally assisting with current and new research study opportunities.

What you will do

We are seeking a compassionate and experienced Part-Time Advanced Practice Provider to join our team and provide specialized care for patients with advanced chronic kidney disease (CKD). In this role, you will be responsible for educating patients on renal replacement therapies (including dialysis modalities and kidney transplant options), facilitating access to vascular surgery and transplant surgery referrals, and coordinating care with nephrology teams to ensure optimal dialysis starts and preemptive transplant options. This position is ideal for an NP with expertise in nephrology and a passion for improving the lives of patients with advanced kidney disease.
