Job Overview This position responsible and accountable for care coordination and medical co-management for patient enrolled in the Complex Care Program. Serves as the accountable nursing leader in the development of the complex care program. Together with the team of clinical staff, prioritizes and delivers patient care within regulations in accordance to their licensure within the Commonwealth of MA. Hours: Full- Time: Days Location: Lowell General, Main Campus Job Description Minimum Qualifications: 1. Bachelor of Science in Nursing (BSN). 2. Registered Nurse (RN) license. 3. Basic Life Support (BLS) or Pediatric Advanced Life Support (PALS) certification 4. Eligible for state and federal controlled substances registration. 5. Extensive clinical experience including project management, program development, and supervision. Preferred Qualifications: 1. Masters Degree 2. Advanced Practice Nursing (APRN) license. 3.
