Position Title: Senior Care Manager

Work Location: Remote - South Carolina (MUST reside in SC; may require travel 25% of time)

Assignment Duration: 4 months

Work Schedule: 8:00 AM - 5:00 PM, Monday to Friday

Position Summary:




  • Transition of Care Case Management for Medicaid members



  • To complete care assessment after hospitalization and provide case management services as needed



  • Working with a great team who collaborates to meet goals




Background & Context:




  • Support Medicaid Line of Business (LOB)



  • Assist in closing HEDIS care gaps



  • Help increase STAR ratings



  • High level of team interaction via email and chat



  • Hardworking environment with strong team communication




Key Responsibilities:




  • Conduct member outreaches after hospital discharge within 10 business days



  • Complete care assessments and provide support post-hospitalization



  • Collaborate with team virtually and occasionally through travel



  • Help improve HEDIS & STAR metrics




Qualification & Experience:




  • Education/Certification Required: Bachelor's Degree, Licensed Health Provider



  • Education Preferred: Master's Degree



  • Licensure Required: Licensed Health Provider (RN, LCSW, LMFT, LPC, PsyD, etc., per HEDIS criteria)



  • Experience Required: Minimum 2 years as a licensed health provider



  • Preferred: Case management experience, psychiatric/mental health background



  • Software Skills: MS Office, Word, Excel, Teams



  • Other Requirements: Must reside in SC; must be willing to travel up to 25% of the time
