The Lead Certified Diabetes Educator (CDE) is responsible for providing medical nutrition therapy, education and medication management to individuals and groups on a variety of diabetes health-related issues including but not limited to: Diabetes, Hyperlipidemia, Pre-diabetes and associated co-morbidities. The Lead CDE will maintain contact with referring physicians to assess patient needs, coordinate care and develop services with the Health Education Department and Diabetes Care Team. The Lead CDE arranges and coordinates the activities of the Health Education department, including the Diabetes Care Team.

Required Qualifications:
  • Bachelor's degree in dietetics or food nutrition OR Bachelor's degree in nursing OR other Bachelor's degree or Master's degree as accepted by the NCBDE (National Certification Board of Diabetes Educators) to obtain CDE credential
  • 2-3 years related experience
  • Individual and group teaching experience
  • Experience in developing and evaluating patient educational materials
  • Experience in working with patient's nutritional needs for Diabetes and related co-morbidities
  • Working with adult populations
  • Certification by the State of Wisconsin - Department of Registration and Licensing for the Registered Dietitian (C.D.) OR Current license by the State of Wisconsin - Registered Nurse
  • Current certification in Diabetes Education (CDE) OR completion of certification within one year of employment
Preferred Qualifications:
  • Master's degree in healthcare related field
  • Outpatient/Ambulatory experience
  • Experience working with Epic EMR
  • Experience working with pediatric population
  • Experience working with ADA recognized DSME curriculum/program
  • Previous supervisory/lead experience
