The Registered Nurse Patient Navigator is a new and exciting role supporting our Comprehensive Dementia Program. The role requires meticulous organization, attention to detail, and collaboration with specialty nurses and providers. The RN Navigator will primarily work with patients undergoing anti-amyloid therapy in the setting of Alzheimer's disease. There is an opportunity for hybrid remote work when training is completed. Four ten-hour shifts or five 8-hour shifts.

Overall, the RN navigator will help navigate the journey of our Alzheimer's patient population who are being treated with anti-amyloid immunotherapy. This role encompasses the need for both clinical and organizational skills with excellent communication skills. The navigator will be a vital part of care for this population and asset to the team as they will communicate and coordinate logistical needs and clinical concerns that may arise as well as aiding in resource identification for patients and caregivers.

Tasks:

* Be familiar with the protocol(s) for both Lecanemab and Donanemab.

* Perform consultation after patient has been consented to initiated medication providing anticipatory guidance on both immediate next steps and what to expect throughout treatment journey.

* Complete and submit benefits investigation paperwork.

* Complete and submit prior authorizations for treatment.

* Track and schedule each patient for the following per protocol/advisement of clinical team:

o Infusion site visits

o MRI's

o Office visits

o PET scans

* Triage calls from patients with clinical concerns.

* Triage calls from infusion centers with acute concerns.

* Complete and submit adverse event reporting for amyloid related imaging abnormalities (ARIA).

* Submit CMS data for patients intially and every 6 months.

    Summary:

    The Registered Nurse - Patient Navigator - Neurology role supports safe patient and family centered care by meeting the specialized needs of patients in the assigned service line as they progress through a wide variety of health care settings. The position is responsible for facilitating care to enhance quality, cost-effective clinical outcomes, and to meet patients' healthcare needs.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Graduate from an accredited School of Nursing required. Bachelor's degree in nursing required or must complete BSN within three years of start date.
    2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required.
    3. Experience: Experience working with patients within the service line preferred, in inpatient, outpatient or community-based care settings.
