At COMHAR, it's our mission to provide health and human services that empower individuals, families and communities to live healthier, self-determined lives. We are currently looking for a COMPASS Program Coordinator to join our team.

COMPASS is a supportive housing program for people who are homeless living with co-occurring mental illness and HIV/AIDS diagnosis. COMPASS provides apartment subsidies as well as support from independent living counselors for those who are committed to treatment.

Duties and Responsibilities:

  • Oversee all program-related activities
  • Facilitate admissions and discharges
  • Calculating member rent based on presented income and related HUD regulations; collecting the rent on a monthly basis
  • Facilitating the completion of unit inspections at least once per year and ensures that all documentation is completed in a timely fashion
  • Generate work orders for maintenance issues; follow-up on submitted work orders to promote completion
  • Coordinating member moves to a new unit when the present apartment is not HUD compliant
  • Maintain history of lease compliance issues for each resident
  • Facilitate an environment that welcomes resident input
  • Whenever possible, work to incorporate the member's family in the decision-making process
  • Recruit, hire, train, supervise, evaluate, correct, and - if necessary - discharge members of the program team
  • Ensure that Supportive Independent Living Counselors and other team members perform all duties necessary to the cases assigned to them in a thorough, appropriate and consistent manner
  • Coordinate staff whenever presented with member-related emergency or crisis
  • Performs regular chart reviews and prepares for internal and external chart audits
  • Complete regular reporting and RFPs (such as HUD's Annual Progress Report, CoC Renewal Application, Quarterly Reports, Annual Progress Report, etc.)
  • Utilize the following data systems: Evolv, Client track (Philadelphia's HMIS), Sage (HUD), as well as others systems deemed necessary.
  • Contributes to the budget process, understands fiscal issues and maintains productivity levels
  • Foster and maintain relationships other housing or AIDS service organizations such as Mission First Housing, DBH, OSH, local shelters, etc.
  • Represents the unit and agency in community (or other agency) meetings, events, outside programming, and outreach efforts
  • Ensure the consistent adherence to COMHAR's policies and procedures
Requirements

Requirement:

  • Experienced working with persons living with HIV/AIDS, Mental illness and knowledge about regional HIV-specific resources preferred
  • Knowledge of support housing service provision and/or case management.
  • Bachelor's degree in social work, psychology, or human services. Master's Degree in social work, psychology, or human service preferred.
  • Supervisory experience.
  • Minimum of two (2) year related experience
