Overview
While the Palliative Care Nurse is responsible for following palliative care patients and families across the entire continuum, his/her primary focus is on coordinating care of patients in the community (clinic, home, LTC) and maintaining open lines of communication between the patient, the healthcare system, and its providers. Palliative Care customers include patients with life threatening or serious and complex illness, their families and caregivers, physicians and the community. The Palliative Care Nurse works closely with the Palliative Care Team in meeting the unique needs of each individual patient and family. The Palliative Care Team members include representation from nursing, social services, pastoral care, the physician and the patient and family.
Qualifications

Responsibilities
- Implements palliative care nursing procedures /processes providing high quality, comprehensive patient care.
- Provides continuity support (by telephone) to community-based Palliative Care patients after initial consult with provider.
- Supports outpatient palliative care clinic by administering assessment tools (ESAS, PPSv2, ECOG, etc.) to patients. (Shared duty with Palliative Care Social Work.)
- Completes the impact assessment form for community-based palliative care patients.
- Provides support and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness.
- Assists in creation and oversight of the care plan in collaboration with the patient/family, primary provider, and the palliative care IDT.
- Assesses on an ongoing basis the effectiveness of the care plan and proactively manages symptoms and goals of care.
- Facilitates patient's or family's abilities to become self-sufficient in adherence to the care plan.
- Assists with special projects and/or other duties as assigned or requested.
- Promotes quality, cost effective care throughout the health continuum.
- Meets with patients in their home setting as needed.
- Coordinates patient care activities and flow of patient information through effective communication with physicians, patients, families, and a variety of health care agencies.

- Conducts systematic follow-up with patients after discharge from UnityPoint Methodist to evaluate if patient goals are met .
- Maintains rapport with patients (telephonically or in the clinic) after patients have a change in care venue.
- Serves as liaison to primary care physicians/clinics keeping all abreast of the palliative care patient's plan/goals of care.
- Keeps the plan/goals of care of Palliative Care patients current
- Demonstrate good verbal and written communication skills with patients, families, healthcare team members and other customers. Verbal communication is clear and precise. Written communication is legible, if handwritten, and done in a timely manner using approved abbreviations.
- Assists with facilitating patient/family conferences as related to palliative care, as appropriate.
- Assists in assessing, developing, and evaluating patient/family educational materials on palliative care.
- Provides internal education to nursing, medical social services, spiritual care and other departments within UnityPoint Clinic and UnityPoint Health. Facilitates education to physicians and medical staff in cooperation with Palliative Care Coordinator.
- Schedule community referrals with families, care providers, and other appropriate resources.
- Adheres to infection control and risk management policies and procedures.

- Supports organizational efforts of establishing and maintaining relationships with communities, benefactors, and other employees.
- Builds partnerships that support and promote Palliative Care strategies.
- Demonstrates evidence of professional growth and development by maintaining a current knowledge palliative care in the acute, post-acute, and community settings.
- Assumes responsibility for being informed about developments in Palliative Care and Hospice nursing.
- Assumes responsibility for own personal and professional development and enhancement of nursing skills.
- Participates in the orientation program for new staff, volunteers and students as assigned.
- Shares education with staff members, clients and caregivers as appropriate.
- Participates in committees, task teams and agency health/educational events as requested.
- Facilitates interdisciplinary teamwork, collaboration and communication among the healthcare team, physicians, clients and caregivers, to ensure integrated efforts toward meeting the client/caregiver needs, goals and satisfaction with services.
- Attends and participates in weekly interdisciplinary team meetings.
- Participates in the development of the Palliative Care interdisciplinary plan of care and attends interdisciplinary team meetings weekly.
- Identifies when supervision and/or peer consultation is indicated and seeks assistance in clinical management of a case when necessary.
- Participates in family conferences.
- Serves as a resource to the medical group and nursing staff in the provision of Palliative Care Services.
- Partners with the healthcare team to achieve the patient care outcomes and goals of care.

Compensation and Benefits
The compensation range for this position is $29per hour - $49.88per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
