Acute Medical Rehabilitation Liaison- Lakewood Medical Center Med Rehab (5 days/wk, 8a-4:30p)

Key Responsibilities:

Serve as the primary point of contact for referral sources (e.g., acute care hospitals, physicians, case managers) regarding patient admissions to the rehab facility.

Evaluate potential patients for medical rehabilitation suitability by reviewing clinical data and collaborating with the rehab clinical team.

Educate patients, families, and healthcare providers about the benefits and services offered by the rehab facility.

Minimum Requirements:

Bachelor's Degree in Nursing, Healthcare Administration, Social Work, or a related field.

Two years of experience in a clinical, case management, or healthcare marketing role.

Possess clinical license in one of the following: RN, PT, OT, SLP, or LCSW

Certified in BCLS (Basic Cardiac Life Support)

In-depth understanding of medical rehabilitation processes and patient needs

Preferred Qualifications:

Prior experience as a liaison or admissions/discharge coordinator in an inpatient rehab facility (IRF) or skilled nursing facility (SNF).
