Job Summary

UW Health in northern Illinois iis seeking a dedicated and compassionate Internal Medicine Advanced Practice Provider (ACNP, AGANP, or PA-C) to join our primary care team. As the preferred primary care provider in Northern Illinois, UW Health offers a patient-centered approach to care, focusing on comprehensive health and wellness for individuals across all stages of life. This position provides an exciting opportunity to work within a collaborative, dynamic environment, serving our diverse patient population.

Your clinical support includes the following:



  • Providing primary care services for a wide range of adult patients with both acute and chronic conditions.
  • Collaborating with a multidisciplinary team that includes physicians, nurses, care coordinators, and specialists to provide holistic care.
  • Managing patient care, conducting physical exams, performing diagnostic tests, and interpreting results.
  • Educating patients and families on managing chronic conditions and promoting preventive care.
  • Participating in patient care planning and follow-up to ensure continuity of care.


Qualifications:



  • Current and valid Illinois license as an Acute Care Nurse Practitioner (ACNP), Adult Gerontology Acute Care Nurse Practitioner (AGANP), or Physician Assistant (PA-C), or ability to apply for license prior to start
  • Experience in Internal Medicine or a primary care setting preferred.
  • Strong clinical decision-making, diagnostic, and communication skills.
