Summary

Provides leadership for Care Management staff providing patient management across the continuum. Responsible for ensuring cost effective, appropriate care. Functions as liaison with the Physician/Medical Team, Clinical departments, Ancillary and Support departments. Responsible for the campus specific fiscal management of the Care Management Department in collaboration with the Vice President, Directors, Team Leaders, and Medical Director. Responsible for the supervision, training and development of the Case Managers and Social Workers, controlling workflow and facilitating seamless operational functioning.

Requirements Educational Requirements

Degree/Diploma Obtained	Program of Study	Required/ Preferred	and/or
Bachelor's	Healthcare	Required	and
Master's		Preferred

Experience Requirements
