Position Title: Care Manager (RN)

Work Location: Remote - SC

Assignment Duration: 4 months (Possibility to extend or convert)

Work Arrangement: Remote, with 25% travel - MUST live in SC

Position Summary:

  To complete care assessment after a hospitalization and provide case management services as needed to members

  
  

Key Responsibilities:

• 
  

  Telephonic outreaches to facilities and members

  
  
• 
  

  Assist facility staff with discharge planning efforts for members

  
  
• 
  

  Works closely together and willing to help when needed to meet attained metrics

  
  

Qualification & Experience:

    Bachelor's Degree

    
    
  • 
    

    Licensed

    
    
  • 
    

    Licensed Health (SC nursing license (compact license will not work))

    
    
  
  
  
  
•

    Master's Degree

    
    
  • 
    

    2+ years as a licensed health provider

    
    
  
  
  
  

Notes:

  Must have the licensed requirements sent in

  Case Management experience, BSN - Nice to haves

  Disqualifier: Not living in SC, No SC license

  Performance indicators: Outreach to members after discharge from hospitalizations within 10 business days
