The Clinical Pharmacist I is a licensed professional who provides safe and optimal comprehensive patient care. This Clinical Pharmacist I (Medication Reconciliation/Transitions of Care) is responsible for maintaining all aspects of medication reconciliation, as well as the transitions of care program which includes review of prior to clinic and prior to admission medication histories, review and counseling of discharge medications, and post discharge follow up. The goal of medication reconciliation and the transitions of care program is to improve communication around the patient's medications, improve patient satisfaction, and reduce medication related adverse events and harm. This individual will supervise technicians and assist with training/education for pharmacy staff. Oversight responsibilities also include but are not limited to overseeing pharmacy technicians' compliance to established processes and productivity goals, as well as documenting key performance metrics. It is expected that this Clinical Pharmacist I will collaborate with colleagues and provide excellent customer service to patients, staff and serve as the medication expert and main point of contact to provide support for providers, staff and patients, when needed.
