Summary The Ambulatory Pharmacy Specialist is responsible for providing clinical pharmacy services for ambulatory patients through accurate medication reconciliation; developing patient care plans including the selection, dosing, and monitoring of drug therapy; promoting medication adherence; and educating patients and other health care providers regarding complexities of drug therapy. Other activities may include ordering labs, initiating new and refill medication orders, and making dose adjustments or drug therapy changes through collaborative care with clinic providers. The position will require working within a multidisciplinary team and supporting patient transitions through the continuum of care. The Ambulatory Pharmacy Specialist supports the development, implementation, and maintenance of programs that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies. Delivers a high level of service to each patient in every interaction, ensuring they perceive Baptist Health's pharmacy as the pharmacy of choice and ultimately impacting their level of success in their therapy. Ensures compliance with pharmacy regulatory standards. This pharmacist may also work on other projects as assigned. The scope and complexity of the position requires a diverse and highly developed skill set including but not limited to: time management, communication, creativity, vision, teamwork, management, leadership, clinical knowledge base/skills, mentorship, change management, scholarship and teaching. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment.
